“WIC” provides nutrition assessment, counseling and supplemental food coupons for “at-risk” pregnant and post-partum women, infants and children up to age 5.   Certified lactation consultants available to help with any breastfeeding needs.

Eligibility:

1) Pregnant and post-partum women, infants and children up to age 5;

2) Reside in DeWitt or Piatt County;

3) Meet income guidelines www.fns.usda.gov/WIC/howtoapply/incomeguidelines.htm; and 

4) Have a medical or nutritional risk.

 

Cost:

                                                  Maternal Child Health (MCH)

Family Case Management

    A program for pregnant women, infants and high risk children.  It assists eligible pregnant women and

    infants with obtaining access to prenatal and well child care, immunizations and screenings.  

     Services provided:     

          - home visits/contacts by a nurse case manager

          - referral for prenatal care

          - referral for well checks and immunizations

          - educational materials about pregnancy, infants and children

          - developmental screenings of infants/young children

          - assistance with All Kids process

          - referral to agencies that may be helpful (food, clothing, shelter, daycare, transportation, etc.) 

 

Healthworks

     Healthworks of Illinois is a collaborative effort of the Department of Children and Family Services (DCFS),

     the Department of Human Services and Department of Public Health.  The program provides a network

     of health care services to wards of DCFS and their children.  It works closely with foster parents

     and caseworkers to ensure that those inf foster care receive appropriate medical care. 

 

Teen Parent Services (TPS)

     Teen Parent Services is a pregram that encourages and assists teens to stay in school.  It helps a teen

     get her/his high school diploma or GED so that the teen can become self-sufficient.  It is operated by

     the Illinois Department of Human Services and is part of the Temporary Assistance for Needy Families

     (TANF) employment and training program.  Direct Services for TPS are provided through the DeWitt-

     Piatt Bi-County Health Department.

    

     DHS requires enrollment if the teen is:

       - age 19 and under

       - receiving TANF cash benefits

       - without a high school diploma or GED

     Teen is eligible to enroll if she/he is:

       - age 19 and under

       - receiving All Kids insurance/medical card

       - in school to obtain a high school diploma or GED

       - pregnant and/or a parent (single or married)

      

     TPS offer many benefits:

       - helps set and work towards goals

       - provides education on topics such as parenting, communication, careers and family health

       - offers incentives toward reaching goals

       - referrals to health care providers, child care, etc.

 

     TPS expects the teen to:

       - attend class in pursuit of a high school diploma or GED certificate

       - meet monthly with the TPS case manager at the health department

       - attend periodic workshops

       - provide weekly school attendance verifications

       - set and review goals toward self-sufficiency
